At Great City Medical, we offer USCIS (U.S. Citizenship and Immigration Services)  medical exams for immigrants as typically required by United States. Our immigration doctors are here to provide compassionate care and analysis during your immigration physical. We will go over any medical conditions, mental illness, mental disorders, and more during your immigration exam.

USCIS Doctors and Immigration Medical Exams



Dr. Mikhail Akselrud is a government-designated civil surgeon authorized to perform the types of medical examinations required of US green card applicants – those individuals seeking to adjust their immigration status to that of a lawful permanent resident. The purpose of the immigration medical exam is to rule out any public health grounds for denying residency requests.

What Does Your Immigration Medical Exam Consist Of?



During your visit with the USCIS immigration doctor, Dr. Akselrud, he’ll conduct a thorough immigration physical exam, including blood tests for communicable diseases such as tuberculosis, gonorrhea, and syphilis. During the physical examination, Dr. Akselrud will also provide you with any required vaccinations and go over your medical history, drug abuse,  and medical records. See the vaccine requirements by applicant age, published by the Centers for Disease Control and Prevention.

  • Great City Medical charges $130 for Civil Surgeon Exam 
  • Syphilis Test – $20
  • Gonorrhea Test – $40
  • Quantifier – $100
  • TD Vaccination – $50
  • MMR Titers – $80
  • MMR Vaccination – $150
  • Varicella Titers: $20
  • Varicella Vaccination – $200
  • Flu Vaccination – $30 (October 1st – March 31st)

*All vaccine fees could be avoided by bringing vaccination records or obtaining them for free at an NYC Health Clinics


Preparing for Your Meeting with the Immigration Doctor


To make the immigration medical exam appointment run smoothly, you’ll want to bring:

  • Report of Medical Examination and Vaccination Record (form i-693)
  • Photo Identification (ID) (government-issued) (valid passport or driver’s license is fine)
  • Immunization record
  • Medical insurance card
  • Payment for the USCIS Doctor visit

You should fill out the I-693 form (adjustment of status) to the extent you are able and bring it with you to the exam. At that point, Dr. Akselrud will use the form to report the results of your medical exam and health condition to the USCIS (United States Citizenship) health official. You will receive a copy of the I-693 form, as well as the original in a sealed envelope, for you to return to USCIS offices in person. Note that the envelope you deliver to USCIS must remain sealed in order to comply with US regulations.
