No. Effective January 22, 2025, USCIS waives any and all requirements that applicants for adjustment of status to that of a lawful permanent resident present documentation on their Form I-693, Report of Immigration Medical Examination and Vaccination Record, that they received the COVID-19 vaccination. USCIS will not issue any Request for Evidence (RFE) or Notice of Intent to Deny (NOID) related to proving a COVID-19 vaccination. USCIS will not deny any adjustment of status application based on the applicant’s failure to present documentation that they received the COVID-19 vaccination.

Previous Immunization Records: Please provide your immunization records before your appointment date. A $50 fee will be applied if you do not have them at the time of your visit. Please notify us when scheduling your appointment if you do not have your records.
The specific vaccinations required for your immigration medical exam depend on your age.
The visit typically takes less than an hour. There is a typical turnaround time of 4 days for your bloodwork results. We will contact you after receiving your results to let you know if any further action is needed or if you are ready to pick up your completed form. At that time, you will receive two envelopes: one sealed for the USCIS and one copy for you or your lawyer.
Great City Medical charges $330 for the Civil Surgeon Exam. This fee includes:
- The physical exam
- Form completion
- Two required blood tests (syphilis and gonorrhea)
- Additional tests may be required based on your individual case. These tests can be completed at an outside facility. However, if you choose to have them performed at our office, additional fees will apply. These tests may include:
- Tetanus (if you have not received one in the last 10 years)
- QuantiFERON-TB Gold
- MMR IgG Titers
- Varicella IgG Titers
- Hepatitis B Titers
- Polio Vaccine
- Flu (during flu season/within the year)
All vaccine fees can be avoided by bringing vaccination records or obtaining them for free at any New York City health clinic or urgent care. We can provide a custom quote for your exam fee if you email us your vaccination records at i693support@greatcitymedical.com.
No, a positive result on any of the bloodwork tests will not prevent you from completing the form. We will work with you to determine the next steps, which may include receiving treatment or further tests to ensure everything is okay before finalizing your form.

Flu vaccine is only required for the USCIS medical exam during flu season. The requirement starts on October 1 and ends on March 31 each year. If your medical examination occurs during this time, you will need to provide proof of flu vaccination or receive the vaccine to meet the requirements.
